After completing the four steps, a tailored patient care plan is generated.
This plan integrates insights gathered from the patient’s health status, current illness, survival expectations, and personal values. By combining this with recommended treatment options from the care team, a personalized and collaborative patient care plan is created.
This can then be transferred to the patient’s medical record to guide treatment in case of deterioration or cardiac arrest.
On completion of the four steps, you can expect:
- Individualized Treatment Options: Specific recommendations based on the patient’s health status and preferences.
- Informed Decision-Making: Incorporating survival expectations for better-informed decisions.
- A Collaborative Approach: The involvement of the care team and patient in the planning process.
