The ASKmeGOC app is a browser-based advanced care planning web application that allows patients, their families, substitute decision-makers, and healthcare providers to work together to develop a care plan for hospital admission that best fits the goals and values of the patient.

How it Works

Step 1: Health Status

Reviews the patient's perceived quality of life, health status, and level of functioning at home before admission in the last year.

Step 2: Current Illness

Identifies a patient’s pre-existing medical conditions and current reason for hospitalization to give an estimation of survival for this admission.

Step 3: Cardiac Arrest

Estimates chance of survival in case of a cardiac arrest while in hospital.

Patient chooses the treatments they would accept in case of deterioration that best fits with their values/goals.

Step 1: Health Status

The Health Status section offers health care providers a comprehensive summary of the patient's self-reported feelings and experiences regarding their health and quality of life over the past year, prior to admission.

Understanding how patients perceive their baseline status gives perspective to the healthcare team and the patient when discussing goals of care, given the current illness and prognosis.

Step 2: Current Illness

The Current Illness section identifies the patient’s pre-existing medical conditions and the current reason for hospitalization, providing health care providers and the patient with important information for care decisions.

With this information, the goals of care software is able to calculate an estimate of survival to discharge for the current admission. This allows more informed advanced care planning decisions with regards to treatment options in case of deterioration.

Step 3: Cardiac Arrest

The Cardiac Arrest section estimates the chance of survival in case of a cardiac arrest while in hospital for the best case (Rhythm VF/VT) and the worst case (Rhythm Asystole/PEA).

Using this information, the patient can make more informed advanced care planning decisions with regards to resuscitation options knowing the estimated outcomes.

Step 4: Values and Goals of Care

The Values and Goals of Care section allows treatment options to align with the patient’s values using an easy to follow scale. The goals of care application has a customizable algorithm to recommend against non-beneficial treatment options to help patients make truly informed decisions.

By incorporating the patient's perspective into care planning, this assessment fosters meaningful discussions between patients and health care providers, ensuring that advanced care planning treatment decisions reflect the individual's wishes and enhance the overall quality of care.

Tailored Patient Care Plan

After completing the four steps, a tailored patient care plan is generated.

This plan integrates insights gathered from the patient’s health status, current illness, survival expectations, and personal values. By combining this with recommended treatment options from the care team, a personalized and collaborative patient care plan is created.

This can then be transferred to the patient’s medical record to guide treatment in case of deterioration or cardiac arrest.

On completion of the four steps, you can expect:
